A Case of Esophageal Cancer with a Metastatic Right No.104 Lymph Node and Suspected Common Carotid Artery Invasion Successfully Resected via a Transcervical-Thoracic Approach

症例は75歳の男性で，嗄声を主訴に前医を受診した．頸部リンパ節腫瘤を指摘され，同部位から生検で扁平上皮癌が検出された．内視鏡検査で胸部上部食道に2型病変を認め，生検結果は扁平上皮癌であった．CTで総頸動脈浸潤を伴う頸部リンパ節転移を認めた．切除不能食道癌［cT4（No.104R-総頸動脈）N2M1a/cStage IVA］に対して化学放射線療法を施行し，原発巣は縮小したがNo.104Rのリンパ節転移巣は遺残を認めた．リンパ節転移巣を含めた局所病変が切除できれば予後が見込めると判断してサルベージ手術の方針とした．頸部リンパ節郭清を安全に施行するために右鎖骨内側切除で術野を確保した．総頸動脈と転移リンパ節は剥離可能で温存し，浸潤を受けていた右内頸静脈・右迷走神経の合併切除を伴う食道切除を施行した．鎖骨切除に伴う肩関節障害は軽度で，鎖骨下領域を術野とする本症例において有用であった．
