現在市販されている複合微量元素製剤は安全性の確認された長期投与可能な薬剤である．しかし中心静脈栄養施行中には半ば強制的に静脈内投与されていることから，おのおのの病態や個人差によって過剰症をきたす可能性を常に孕んでいる．実際に本邦ではこれまでにマンガン過剰症を当院から報告し，是正されている歴史もある． 今回，当科の小児および成人の長期在宅中心静脈栄養（以下，HPN）患者症例における血中微量元素濃度を後方視的に検討したところ，銅，亜鉛，マンガンの濃度は正常にもかかわらず，血清鉄や血清フェリチン値が異常高値，つまり鉄過剰状態の症例が散見された． このような経験から血清鉄やフェリチンが異常高値の症例に対しては，市販の微量元素製剤から鉄非含有の微量元素製剤の使用に切り替えている．当院における鉄非含有微量元素製剤の取り組みについても報告する．
