Rate of Isolation of Legionella pneumophila in Patients at Kawasaki City, and a Cluster of ST1346

レジオネラ症はレジオネラ属菌を原因とする感染症であり,国内の患者の報告数は年々増加しているが,感染源不明の事例が多い.本研究では2014年1月から2019年12月までに川崎市で分離されたLegionella pneumophila SG1計28株について感染経路の調査を目的として,Sequence-Based Typing(SBT)法による解析を実施した.その結果ST1346が5株,ST89,ST92及びST142が2株ずつ検出され,残りの17株はそれぞれ異なるSTとなった.最も多く分離されたST1346は5株中4株が2016年11月に短期間に集中して分離されており,ST1346の5株目は11カ月後の2018年10月に分離された.ST1346の5株について全ゲノム解析を行い,pairwise single-nucleotide variant(SNV)の算出を行ったところ,5株間のSNV数は3~6SNVであり,非常に近縁の株であることが明らかとなった.継続的にレジオネラ症患者から菌株を分離しSBT法を実施することで,同一STの集積が確認されSNV解析から同一の感染源に由来することが示唆された.この解析を実施し,疫学調査に反映させることで,集団感染を探知できる可能性がある.
