Effects of Combining Acupuncture and Kampo Medicine in Patients Trying to Conceive over the Course of Two or more In Vitro Fertilization Treatments

【目的】IVF （in vitro fertilization） を繰り返し実施しても妊娠に至らず治療が難渋する例は多い。 こうした難治性不妊症患者を対象に、 鍼と漢方薬の併用治療が妊娠に与える影響について検討した報告は、 諸外国を併せても殆ど見当たらない。 そこで今回は、 2回以上IVFを実施しても妊娠・出産に至らなかった難治性不妊症患者5症例に対する鍼と漢方薬の併用治療効果について検討した。 【症例】症例1：40歳女性、 不妊症治療歴12カ月。 X－1年に自然妊娠したが流産した。 約半年後に体外受精を開始し、 2回目の胚移植で妊娠するも流産した。 症例2：46歳女性、 不妊治療歴36カ月。 体外受精を6回実施したが妊娠に至らなかった。 症例3：37歳女性、 不妊治療歴24カ月。 人工受精を5回、 体外受精を3回実施したが妊娠に至らなかった。 症例4：26歳女性、 不妊治療歴60カ月。 体外受精を7回実施し、 5回流産した。 症例5：38歳女性。 不妊治療歴108カ月。 長期的な不妊治療により頸肩凝りを自覚していた。 症状が増悪した為、 人工受精の開始と共に鍼治療を開始した。 【結果】症例1、 症例3、 症例4、 症例5は妊娠・出産に至った。 症例2は妊娠に至らず終診となった。 【考察・結論】難治性不妊症患者に対する鍼と漢方薬の併用治療は、 継続的に実施し全身状態を良好に維持する事で、 妊娠に期待すべき効果が得られた。

症候性急性期脳梗塞の連続130例について気血水スコアを用い気虚，気鬱，気逆，血虚，瘀血，水滞の有無を評価し，同年代のその他の神経疾患連続93例を比較対照として比較検討した。多変量解析で急性期脳梗塞では他の神経疾患と比較して瘀血との関連性が最も高く，調整済みオッズ比（95%信頼区間）は4.6（2.45-8.91）だった。交絡因子となる性別を層別解析で調整しても瘀血の調整済みオッズ比（95%信頼区間）は男性で7.46（3.02-20.25），女性で2．63（1.02-7.11）と最も高かった。瘀血の素点（中央値，四分位範囲）は急性期脳梗塞（24点，18-33点）で他の神経疾患（16点，9-23点）より重症度が高かった。急性期脳梗塞群で臨床病型，入院時の重症度，性別との関連を検討したが瘀血の割合が最大だった。症候性の急性期脳梗塞では他の神経疾患と比較して瘀血との関連性が最も高いことが示唆された。
