A case of alcoholic liver cirrhosis complicated with hepatic IgA nephropathy and massive chylous ascites

症例は58歳，男性．常習飲酒家．腹部膨満感と下肢浮腫のため当院紹介となった．CT画像より肝の形態は肝硬変であり，著明な腹水を認めた．血液検査所見はアルコール性肝障害の他，蛋白尿（1.87 g/日）と血尿を伴う腎障害を認めた．腹水は乳糜腹水であり，色調は乳白色で腹水中のトリグリセリド値は高値であった．入院当初は利尿剤への反応は不良であったが，肝機能の改善に伴い蛋白尿は減少し腹水は消失した．第35病日に行った腎生検ではメサンギウム細胞の増生とメサンギウム領域へのIgAの沈着を認め，IgA腎症と診断した．本症例は背景にアルコール性肝硬変があり肝性IgA腎症と診断した．退院後も禁酒が維持され，肝機能，腎機能は増悪なく安定している．肝障害患者の蛋白尿と血尿を見た場合，肝性IgA腎症は鑑別診断の一つとなりえる．
