腎機能障害を有する患者にセフェピム（CFPM）を7週間投与し発症したCFPM脳症に対し，血液透析（HD）が有効であった1例を経験した．化膿性脊椎炎に対してCFPMを投与され，第50病日JCS 10の意識障害とCr 8.5mg/dLの急性腎障害を認めた．脳波で3相波を認め，長期のCFPM投与歴からCFPM脳症を疑い，CFPMの透析性が高いことを考慮しHDを第51病日から3日間連日施行した．第53病日にJCS 0に改善し，臨床的にCFPM脳症と診断した．CFPM血中濃度を解析した結果，HD前の第50病日174.1 μg/mL（推奨治療濃度46 μg/mL前後），第53病日12.3 μg/mLと低下していた．腎排泄性のCFPMを投与中の腎機能障害患者では，意識障害の原因にCFPM脳症を鑑別にあげるべきである．また，CFPMは透析性が高いことからHDがCFPM脳症の効果的な治療手段となり得る．
