The great results of our unique intrahospital cooperation alert system for positive hepatitis C test results

当院では肝炎ウイルス検査について適切な結果説明と介入を行うため，多職種横断的な院内連携システムを構築した．本システムの特長はアラートへの返答時に電子カルテ上の所定テンプレート作成を必須とした点と，適切な対応が行われるまでコメディカルによるアラートが繰り返し行われる点である．システムの有効性とその経時的変化を評価するため非消化器内科医によりオーダーされたHCV抗体検査陽性者を前期155例（2017年9月から2018年8月）と後期121例（2018年9月から2019年8月）に分け検討した．患者に対する最終介入率は有意な経時的上昇があり（前期88.4％vs後期 98.3％，p = 0.002），適切な検査・治療へとつなげることができた．当院の独自性の高いアラートシステムはコメディカルによる勧告を繰り返すことで消化器内科医師の負担を殆ど増やさずに肝炎患者を抽出し適切な治療に結びつける有用なシステムである．
