Carotid artery stenting with double-layer micromesh stent (CASPER stent)—the time-related changes in residual ulceration outside the stent

潰瘍形成プラークに対する頚動脈ステント留置術後には，ステント外に造影剤が漏出するステント外残存潰瘍（residual ulceration: RU）を認めることが多い．RUは半年後に約半数が消失するが，術後脳塞栓症の原因となる可能性について報告されている．本研究では，潰瘍病変に対してCASPER stentを使用した場合のRU消失率および塞栓性合併症の発生率を検討した．2021年4月から2022年3月までの期間で，潰瘍形成を認めた11症例を対象とした．全例にCASPER stentを用い，術後3カ月時点のDSA所見を評価した．10例にRUを認め，3カ月後のRU消失率は78%であった．1年間の追跡で症候性脳梗塞の発生例はなかった．潰瘍病変に対するCASでは，CASPER stentの使用によりRUの高い消失率が得られた．
