「肺血栓塞栓症・深部静脈血栓症および肺高血圧症に関するガイドライン」では静脈血栓塞栓症（venous thromboembolism: VTE）の診断では，重症度，誘因を評価することが明確化された．肺血栓塞栓症（pulmonary thromboembolism: PTE）では臨床所見（検査前臨床的確率スコア含む），否定診断にD-dimer，否定できない患者に確定診断にCTを用いる．確定されたPTEは臨床所見，PESIスコア，右心負荷，バイオマーカーで高，中高，中低，低リスクと重症度診断する．慢性期のPTEの慢性血栓塞栓性肺高血圧症の診断に肺換気血流シンチグラムが有用である．深部静脈血栓症（deep vein thrombosis: DVT）では臨床所見（検査前臨床的確率スコア含む），否定診断にD-dimer，否定できない患者に確定診断に下肢静脈エコーを用いる．確定されたDVTは症状評価と下大静脈，腸骨静脈，大腿静脈，膝窩静脈に血栓が存在する中枢型DVTと下腿静脈のみに血栓が存在する末梢型DVTと分類する．さらに抗凝固療法の継続期間を決定するために “誘因” も評価する．
