Unexpected Pregnancy in a 42-Year-Old Pregnant Woman after a Polymyomectomy Whose Fibromatous Nuclei Mass Weighed 4200 Grams at the Mali Hospital

Introduction: Fibroids (myomas) affect 20% to 25% of women of childbearing age and are 3 to 9 times more common in black women. A delay in diagnosis due to the reluctance of some patients to conventional medicine leads to excessive volumes of fibroids, the ideal treatment of which in our context remains polymyomectomy (enucleation of several fibroid nuclei). 3 The objective was to report the case of an unexpected late pregnancy post-polymyomectomy in a multi-procedure in the gynecology department of the Mali Hospital. Observation: it was a 39-year-old patient, a housewife, nulligeste, and with no known medical history, who had consulted the gynecology department for an abdominal-pelvic mass. Given the progressive increase in the volume of the abdomen, she decided to consult. The physical examination confirmed the presence of a large abdominal-pelvic mass measuring 36 cm in length along its longitudinal axis with the tape measure. The rest of the physical and biological examination was unremarkable. Ultrasound and abdominopelvic CT confirmed the presence of a large and empty polymyomatous uterus. She had undergone a polymyomectomy by laparotomy. An extirpation in two blocks of the fibromatous nuclei, each measuring between 15 cm and a pedicled nucleus 6 cm in diameter. Three years later, at the age of 42, she consulted for a pregnancy of 20 weeks of amenorrhea, without fertility treatment, and confirmed by obstetric ultrasound. She gave birth by caesarean section at 37 weeks, to a healthy male newborn. Conclusion: Fibroids can be responsible for infertility, especially in elderly nulligestures. Treatment is essentially surgical. The conception of a pregnancy can occur either therapeutically or spontaneously.
