Effect of Integrated Nursing Care Innovation on Treatment Adherence in Diabetes Mellitus Patients in the Community

Diabetes mellitus (DM) is a chronic disease requiring lifelong treatment, yet medication adherence among patients in primary care remains low due to fragmented continuity of care, limited structured education, and inadequate community-based monitoring. This study aimed to evaluate the effect of an integrated nursing care innovation on treatment adherence among patients with type 2 diabetes in the working area of Sidomulyo Community Health Center, Pekanbaru. A quantitative quasi-experimental study with a pretest–posttest control group design involved 76 respondents enrolled in the Chronic Disease Management Program (Prolanis). Participants were selected using purposive sampling and assigned to intervention (n=38) and control (n=38) groups. Medication adherence was measured using the Morisky Medication Adherence Scale (MMAS-8), while implementation of integrated nursing care was monitored using an observation sheet. Data were analyzed using descriptive statistics, Paired Sample t-Test, and Independent Sample t-Test. The intervention group demonstrated a substantial increase in mean adherence score from 12.4 to 18.6, compared with a slight increase from 12.1 to 13.0 in the control group. Significant improvement occurred within the intervention group (p<0.001), with a significant difference between groups (p=0.001). Integrated nursing care effectively improved treatment adherence and may strengthen long-term diabetes management through nurse training, community health volunteers, scheduled home visits, and reminder systems.
