HAZARD-RANK: De-identified Hospital Occupational Hazard Register

De-identified occupational hazard register of a single hospital occupational health and safety unit, comprising 1,479 free-text hazard entries across 28 coded organizational units, together with the held-out test partition, the 180 blind expert-comparison pairs, the ratings of four independent expert raters, and the derived consensus gold sets used to validate an automated risk-prioritization model. Hazard descriptions, expected harm, and proposed control actions are recorded in Turkish free text. Hospital unit names are replaced with coded labels; the code-to-name mapping is retained by the author. The register contains no personal identifiers, and the responsible-party column holds organizational roles only. Files in this record are under restricted access because the free-text fields describe conditions in one identifiable organization. The record page and metadata remain public and files are released to named requesters through Zenodo's access request mechanism, for non-commercial verification and replication purposes. This dataset supports the manuscript "Weakly Supervised Pairwise Preference Learning for Explainable Occupational Hazard Risk Prioritization in Hospitals" (Taşkın, 2026).
