Diagnosis, mechanisms, and integrated Chinese-Western medicine prevention of oxaliplatin-induced peripheral neuropathy

Oxaliplatin (OHP) is a third-generation platinum-based chemotherapeutic agent that frequently causes adverse effects, including chemotherapy-related gastrointestinal reactions, hypersensitivity, myelosuppression, and peripheral neurotoxicity.Among these adverse effects, OHP-induced peripheral neurotoxicity (OIPN) is the primary dose-limiting toxicity.OIPN significantly impairs patients' quality of life and interferes with the continuous administration of OHP, often resulting in dose reduction or treatment discontinuation.Given the current lack of effective monitoring and preventive strategies, this review constructs a complete three-stage integrated Chinese-Western medicine prevention system stratified by acute and chronic OIPN, proposes scenario-specific combined intervention regimens and interprets their synergistic molecular mechanisms.Integrated traditional Chinese and Western medicine demonstrates unique advantages in achieving synergistic prevention of OIPN.This combined strategy leverages precise risk assessment and targeted neuroprotection from Western medicine, while incorporating traditional Chinese medicine principles such as syndrome differentiation and preventive treatment.By overcoming limitations inherent to a single medical approach, this integrative strategy significantly reduces the incidence and severity of OIPN, improves patients' quality of life, and ensures smooth chemotherapy implementation.Thus, it opens a novel pathway for OIPN prevention in clinical practice.This review summarizes recent research advances in diagnostic methods, pathogenic mechanisms, and integrated Chinese-Western mecicine for the synergistic prevention of OIPN, while highlighting directions for future research.
