Empowering carers to partner with healthcare professionals in the prevention and early management of delirium for patients undergoing cardiac surgery

Early recognition of delirium is essential because it enables timely identification and management of underlying causes, reduces complications and length of stay, and improves recovery outcomes. Carers often notice subtle cognitive and behavioural changes earlier than clinicians because of their familiarity with the patient’s baseline, making their observations crucial for early recognition and timely prompt intervention in delirium. However, most current delirium prevention programs focus on clinicians and patients rather than on positioning carers as proactive partners before surgery. An integrative review will be conducted to synthesise findings from current research reporting the prevention and early management of delirium for patients undergoing cardiac surgery. The purpose of the integrative review is to describe the preadmission assessment processes that address the risk for delirium for patients undergoing cardiac surgery and identify gaps in delirium education, risk recognition and carer involvement. The expected outcome is the identification of gaps in current practice, particularly in relation to carer engagement and preoperative delirium education. These findings will inform carer education interventions aimed at strengthening early recognition and prevention of delirium in preoperative cardiac settings.
