Integrative Korean Medicine Treatment Combined with Ultrasound-Guided High-Dose Shinbaro 2 Pharmacopuncture for Severe Lumbar Spinal Stenosis Accompanied by Abdominal Obesity: Three Case Reports

This study aimed to report the clinical course and safety of integrative Korean medicine treatment combined with ultrasound-guided high-dose Shinbaro 2 pharmacopuncture in three patients with severe lumbar spinal stenosis accompanied by abdominal obesity.Three patients diagnosed with severe lumbar spinal stenosis accompanied by abdominal obesity received integrative Korean medicine treatment, including ultrasound-guided high-dose Shinbaro 2 pharmacopuncture, acupuncture, electroacupuncture, infrared irradiation, chuna therapy, and herbal medicine when indicated.Abdominal obesity was evaluated using body mass index, waist circumference, and visceral fat area measured by body composition analysis.Clinical outcomes included the numeric rating scale, oswestry disability index, EuroQol-5 dimension-5 level, pain-free walking time, patient global impression of change, and adverse events.Low back pain and radiating leg pain improved in all three cases.The mean numeric rating scale score for low back pain and radiating leg pain decreased from 6.7 to 2.7.The mean oswestry disability index score improved from 63.70 to 16.30, and the mean EuroQol-5 dimension-5 level index score improved from 0.448 to 0.952.Pain-free walking time improved from less than 1 to 10 minutes before treatment to more than 20 to 60 minutes after treatment.Patient global impression of change scores were 1, 1, and 2, respectively.No serious adverse events were reported.These cases suggest that integrative Korean medicine treatment combined with ultrasound-guided high-dose Shinbaro 2 pharmacopuncture may be a feasible non-surgical treatment option for improving pain, function, and walking ability in patients with severe lumbar spinal stenosis accompanied by abdominal obesity.(
