Background/Introduction Intracranial Mechanical Thrombectomy (MT) may fail in up to 30% of cases due to underlying intracranial stenosis or refractory thrombus. In these situations, bail-out stenting represents a potential strategy to achieve permanent recanalization and improve functional outcomes. Objective/Purpose The RECHRUT study evaluates the safety and effectiveness of the CREDO® heal stent after unsuccessful thrombectomy in patients with suspected intracranial stenosis. Methods/Case Description RECHRUT is a prospective, single-arm, multicentre, open-label international PMCF study for patients with acute ischemic stroke and unsuccessful MT. Technical success was defined as achieving TICI 2b–3 at the end of the procedure; clinical success as an mRS of 0–2 at 90 (±20) days. Results/Findings Since March 2023, 121 patients have been enrolled across 20 European centers. In an interim analysis of 50 patients, stenting was typically performed after two MT passes, mainly in M1 lesions. For these 50 cases, mean age was 68 years and baseline mRS was 3.2. Recanalization success (mTICI 2b–3) was achieved in 98%. Mean mRS at discharge was 2; 90-day data were available for 39/50 patients, with 56% achieving mRS 0–2. Mean stenosis improved from 83% pre-treatment to 25.5% post-PTA/stenting. Four patients died, three of them from pneumonia. No recurrent occlusions were observed. Tirofiban was administered in 98% of cases; 54% received additional ASA. Five patients (5/50) were on oral anticoagulants as standard medication before admission. Discussion/Conclusions/Learning Points Early interim findings show technical success and promising clinical outcomes, supporting the potential role of CREDO® heal as a viable rescue stenting option after failed MT.
