Background/Introduction Cerebral vasospasm (CV) and the resulting delayed cerebral ischemia (DCI) are major complications of aneurysmal subarachnoid hemorrhage (SAH) and remain key contributors to morbidity and mortality, even after successful aneurysm treatment. Current endovascular strategies for CV are associated with variable efficacy and procedural risk, including vessel rupture. Angioplasty using the Comaneci device may offer a safer, atraumatic alternative; however, it has not been systematically evaluated in prospective controlled studies. Objective/Purpose We hypothesized that angioplasty with the Comaneci device is a safe and effective treatment for CV following SAH. Methods/Case Description This was a single-arm, open-label, multicenter, core-laboratory–controlled, prospective registry study. Adults (≥18 years) presenting with ≥50% cerebral vasospasm after SAH were enrolled. All patients underwent endovascular treatment using the Comaneci device as a first-line intervention in the affected vessel segments. The primary efficacy endpoint was procedural success, defined as a ≥50% increase in vessel caliber on digital subtraction angiography (DSA) versus baseline. Primary safety endpoints included intraoperative and postprocedural complications. Radiological outcomes were assessed using a standardized four-grade scale. Results/Findings Fourteen patients (50% female; mean age, 48.2 years) from six participating centers were included. In total, 34 vessel segments were treated. Procedural success was achieved in 97.1% (33/34) of segments. One intraoperative complication, possibly device-related, was observed (worsening angiographic vasospasm). Vasospasm recurrence occurred in 9/34 vessels; no retreatment was required. No ischemic infarction was detected in territories of Comaneci-treated vessels. At 30-day follow-up, modified Rankin Scale (mRS) scores improved by ≥1 point in 9 of 12 evaluable patients (75%). Discussion/Conclusions/Learning Points In conclusion, Comaneci-assisted angioplasty achieved a high rate of angiographic success with a low complication rate and a trend toward improved clinical outcomes. These findings suggest that the Comaneci device may represent a safe and effective therapeutic option for refractory cerebral vasospasm following aneurysmal SAH.
