Extracorporeal membrane oxygenation (ECMO) oxygenators' lifespans show a dynamic link between patient pathophysiology, device design, and bedside control.Hypercoagulability, systemic inflammation, hemolysis, and illness severity promote membrane fouling, intramembrane thrombosis, and a decline in gas-exchange efficiency.These patient-controlled factors overlap with membrane material, surface coating, pump design, circuitry, and anticoagulation regimens to cause gradual or rapid deterioration.Clinical practice has been proactive since acknowledging this relationship.Trend-based monitoring of transmembrane pressure (DP), pre-and post-oxygenator blood gases, fibrinogen, D-dimer, platelets, and hemolysis indicators helps identify high-risk patients and schedule elective oxygenator exchange.We assess risk characteristics like COVID-19 acute respiratory distress syndrome (ARDS) and sepsis-induced systemic inflammatory response syndrome (SIRS), and modify anticoagulation individually.Polymethylpentene (PMP) membranes, biocompatible coatings, compact circuitry, and low-shear pumps also help increase oxygenator life.Despite advancements, understanding of oxygenator failure and projected comparative data are largely fragmented.Standardized outcomes, multicenter registries, biomaterial innovation, real-time monitoring, and artificial intelligence (AI)-based predictions should be applied to improve ECMO safety.Clinical significance: Recognition of oxygenator deterioration at the least invasive levels, using data on DP patterns, gas-exchange levels, and coagulation and hemolysis biomarkers, might help clinicians shift from reactivity to proactivity in managing the situation, thus enabling safer elective changes without catastrophic changes in circuit structure.Combining individual risk factors of patients with a considerate choice of the device and specific anticoagulation methods can make a huge contribution to the extension of oxygenator durability and better ECMO results.
