Association of inflammatory markers (Inflammatory Burden Index, Systemic Inflammation Response Index, Systemic Immune-Inflammation Index) and modified Glasgow Prognostic Score with 30- and 90-day mortality in a level 3 surgical intensive care unit

Aims: We aimed to examine the relationship between systemic inflammatory markers and mGPS with 30- and 90-day mortality in critically ill patients who underwent various types of surgery (abdominal, neurosurgical, orthopedic, and urogynecological surgeries) and were followed up in a tertiary surgical intensive care unit. Methods: This retrospective study included patients who were postoperatively followed in the tertiary surgical intensive care unit between January 2021-2025. A total of 455 patients were included in the study. Data regarding patients’ age, comorbidities, length of hospital stay, length of intensive care unit stay, preoperative 24-hour inflammatory parameters (leukocyte, neutrophil, platelet, lymphocyte, monocyte, CRP, albumin, procalcitonin), and APACHE II and SAPS II scores at intensive care unit admission were collected. Composite inflammatory biomarkers (IBI, SIRI, SII, mGPS) were calculated using formulas. Results: A total of 455 postoperative patients were included in the study. Among them, 270 (59.3%) were male, and the mean age was 69.69?±?17.18 years. Of all patients, 193 (42.2%) underwent abdominal surgery, 74 (16.3%) underwent neurosurgery, 140(30.8%) underwent orthopedic surgery, and 48 (10.5%) underwent urogynecological surgery. Among the inflammatory indices, the Inflammatory Burden Index (p = 0.001 for 30-day, p < 0.001 for 90-day), Neutrophil-to-Lymphocyte Ratio(p = 0.029 for 30-day, p = 0.033 for 90-day), and Modified Glasgow Prognostic Score (p = 0.005 for 30-day, p < 0.001 for 90-day) were significant predictors of both short- and mid-term mortality. Conclusion: Among postoperative surgical patients, Inflammatory Burden Index and Modified Glasgow Prognostic Score are significant predictors of 30/90 day mortality.
