Neurophysiological Assessment and Functional Reconstruction in Traumatic Peripheral Nerve Injuries of the Extremities: An Integrated Approach to Diagnosis, Timing, and Recovery

Traumatic peripheral nerve injuries of the extremities represent a significant cause of motor, sensory, and functional impairment, particularly when diagnosis or reconstructive management is delayed. This study analyzed the role of neurophysiological assessment and functional reconstruction in the management of traumatic peripheral nerve injuries through an integrative review of clinical and surgical evidence. The analysis focused on mechanisms of nerve injury, clinical examination, nerve conduction studies, needle electromyography, peripheral nerve ultrasound, magnetic resonance neurography, timing of intervention, direct nerve repair, nerve grafting, nerve transfers, and rehabilitation. The findings showed that clinical and neurophysiological assessment constituted the most frequently represented diagnostic components, while imaging provided complementary structural information regarding nerve continuity, neuroma formation, compression, and fascicular abnormalities. Regarding reconstruction, nerve grafting, direct repair, and nerve transfer were the most commonly addressed strategies. Injury severity, anatomical level, timing of intervention, regenerative distance, evidence of reinnervation, and target-muscle condition emerged as the principal factors associated with reconstructive decision-making and functional prognosis. The evidence indicates that no isolated diagnostic modality or surgical technique is sufficient for all traumatic nerve injuries. Instead, serial neurological examination, appropriately timed electrodiagnostic testing, complementary imaging, and individualized reconstructive planning should be integrated within the same therapeutic pathway. Early recognition of severe lesions and timely referral for reconstruction may preserve opportunities for motor and sensory recovery before prolonged denervation compromises the biological capacity of the target tissues. Functional rehabilitation remains essential for translating neural regeneration into useful movement, sensation, and independence.
