Fulminant Japanese encephalitis in pregnancy: A rare maternal–fetal catastrophe

Japanese encephalitis (JE), the leading cause of viral encephalitis in Asia, is rarely reported during pregnancy but carries high maternal and fetal risk. We describe a 26-year-old gravida 2 para 1 at 15 weeks of gestation who developed fulminant JE with rapid neurological decline, hepatic dysfunction, and sepsis-like episodes. Despite intensive care, ventilatory support, tracheostomy, and nutritional management, she suffered spontaneous abortion and maternal death. This case underscores the devastating course of maternal JE, highlighting the need for early recognition, prompt airway protection, and heightened vigilance in endemic regions.
