Objectives: The aim of our study was to analyze the levels of angiogenic markers in a cohort of women with uncomplicated twin pregnancies and to compare them with a cohort of twin pregnancies with pathological outcomes.Methods: This prospective study was carried out in 2020-2025.Maternal blood samples were analyzed using a standardized immunoassay to measure proangiogenic (PlGF) and antiangiogenic (sFlt-1) markers.Based on their outcomes, pregnancies were classified into the complication and the control group.A total of 285 twin pregnancies were included, of which 140 were dichorionic (DC) and 145 were monochorionic (MC).Complications were observed in 208 pregnancies, while 77 were uncomplicated.The proportion of complications was higher in MC pregnancies (77.8%) compared with DC pregnancies (68.6%).Results: Table 1 presents biochemical markers for each twin pregnancy category before and after 33 weeks of gestation.Results are expressed as absolute values and as multiples of the median (MoM), with uncomplicated DC pregnancies serving as the reference standard for MoM calculation.In uncomplicated DC pregnancies, sFlt-1/PlGFratio increases by 30,36% per week, vs 17,35% in complicated DC and 13,72% in complicated MC pregnancies.Conclusions: The sFlt-1/PlGF ratio represents a valuable tool for risk stratification in twin pregnancies with suspected preeclampsia or fetal growth restriction.Its application may help identify patients requiring expedited delivery and support individualized clinical decision-making.We have established a MoM-based normative reference for angiogenic markers in DC twin pregnancies, implemented in the form of a risk calculator incorporating maternal age, BMI, conception type, blood pressure, proteinuria and marker levels at specific gestational weeks.EP12.
