Suprapatellar nailing tibia - an upgrade in alleviating the difficulties of infra patellar nailing tibia: A surgeon’s perspective

Background: Tibia fractures at are a common occurrence in trauma surgery. Different levels of fracture tibia pose different challenges in implant selection. With utilization of interlocking nailing of tibia through infrapatellar approach most of the shaft fractures are managed but comminuted, segmental, proximal and distal fractures of tibia poses great difficulty for these nail as it may cause malreduction and malalignment which can lead to implant failure or revision. In this study we aimed to evaluate the clinicoradiological outcomes in all level of tibia fracture which are managed with suprapatellar nailing of tibia.Methodology: In this study, consecutive cases (45) of different level of tibia fracture in an apex tertiary care trauma center are included. The subjects received same treatment protocols and post-operative clinicoradiological function is evaluated. Analysis: Accurate entry point is obtained in all cases with c arm assist, average surgical time was 46 minutes, all the levels of fractures united on follow-up, no evidence of any malreduction or malalignment on these cases, no patients complained of anterior knee pain.Conclusion: We recommend suprapatellar nailing of tibia in all levels of tibia fractures as it significantly reduce the rate of nonunion, malalignment, revision surgeries, anterior knee pain.
