Prediabetes Remission as a Pragmatic and Scalable Prevention Endpoint in Low-resource Settings

Prediabetes is a major global health challenge and a precursor not only to type 2 diabetes (T2D) but also to increased cardiovascular morbidity and mortality.Traditional prevention strategies have focused primarily on delaying progression to diabetes.However, emerging evidence suggests that sustained regression from prediabetes to normoglycemia-prediabetes remission-may represent a clinically meaningful and measurable prevention endpoint.Long-term follow-up from landmark studies demonstrates that individuals who revert to normoglycemia experience sustained reductions in diabetes incidence and, more recently, lower long-term risk of cardiovascular death and hospitalization for heart failure.Community-based and lifestyle intervention trials indicate that remission is achievable in real-world settings, including resourceconstrained environments.Recognizing remission as a therapeutic target reframes prevention from risk containment to restoration of metabolic health.For clinicians, this raises important considerations regarding operational definitions, durability of remission, monitoring strategies, and long-term cardiovascular risk management.Integrating remission into routine care pathways may strengthen patient engagement, improve cardiometabolic outcomes, and provide a pragmatic framework for scalable diabetes prevention.
