Objectives This paper presents the sixth revision of the Korean Medication Algorithm Project for Bipolar Disorder (KMAP-BP2026), focusing on maintenance treatment.The update reflects the recent clinical shifts toward atypical antipsychotics (AAP) and safety-centered pharmacological strategies.Methods A consensus-based survey was conducted among 109 Korean experts (67 adult and 42 child/adolescent specialists).Using a nine-point scale, the treatment options were categorized into first-line (7-9), second-line (4-6), and third-line (1-3) strategies. ResultsFor maintenance after a manic episode, first-line recommendations include mood stabilizer (MS) monotherapy, AAP monotherapy, MS+AAP combination, and MS+lamotrigine (LTG).Aripiprazole remains a core agent because of its superior efficacy and safety profile.Following a depressive episode, various combinations, AAP monotherapy, and LTG monotherapy were prioritized.
