Manganese is needed in trace amounts for physiological functions, such as enzymatic activity. However, chronic exposure to manganese, which has been reported in occupational settings such as mining, can lead to manganism. While Manganism shares similar symptoms (e.g bradykinesia, rigidity, and postural instability) with Parkinson's disease (PD), its pathophysiology is distinct from PD. Contrary to PD, globus pallidus and striatum are the primary damaged structures in manganism. Differences in dopaminergic dysfunction, and neuronal degeneration between the two conditions have been reported. Importantly, manganism often shows poor response to levodopa, underscoring the need for differential diagnosis and tailored management strategies. This write-up points out the basis of manganese neurotoxicity, its clinical manifestations, diagnostic challenges, and therapeutic approaches, while also examining the implications for environmental health and occupational safety. the chapter aims to enhance understanding of manganese and Parkinsonism.
