Association between Cardiovascular Health and Cognitive Performance: A Multimodal Study Using an Eye-Tracking-Based Digital Biomarker (ViewMind Atlas®)

This observational, analytical, cross-sectional study will examine the association between vascular aging, cardiovascular health, and cognitive performance in 148 community-dwelling adults aged 45 years or older, users of the Brazilian Unified Health System (SUS) in Indaiatuba, São Paulo, Brazil. Participants without established neurological disease will undergo blood-pressure and waist-circumference measurement, transthoracic echocardiography, and carotid ultrasonography for carotid intima-media thickness (CIMT) and vascular aging. Cognitive performance will be assessed with ViewMind Atlas®, an eye-tracking-based digital biomarker, and with the Montreal Cognitive Assessment (MoCA). In a second visit, participants will undergo resting-state functional MRI at 3 Tesla, with dynamic functional connectivity characterized by Leading Eigenvector Dynamics Analysis (LEiDA). The primary hypothesis is that greater vascular aging, indexed by CIMT, is associated with poorer cognitive performance on the ViewMind Atlas® global score. A secondary, exploratory hypothesis is that dynamic functional connectivity partially mediates the association between cardiovascular health and cognition. The study is non-interventional and has been submitted to the Research Ethics Committee of Centro Universitário Max Planck (UniMAX).
