Management of glaucoma in high myopia: Guidelines from the French Glaucoma Society (SFG) and the French-speaking Retina Specialists’ Club (CFSR)

Glaucoma is the leading cause of irreversible blindness worldwide. High myopia, an independent risk factor for this disease, adds complexity to its diagnosis, follow-up, and medical and surgical management. These guidelines represent a consensus of French glaucoma and retina experts on the management of glaucoma in high myopia. The therapeutic strategy and follow-up modalities are similar to those used for primary open-angle glaucoma, the goal being to achieve a target intraocular pressure (IOP) that prevents the onset or worsening of glaucomatous damage. In case of elevated IOP and diagnostic uncertainty due to extensive chorioretinal alterations interfering with the fundus assessment and interpretation of tests, IOP-lowering treatment may be initiated to prevent the risk of irreversible glaucomatous damage in addition to that already associated with high myopia. The increased incidence of postoperative complications should be taken into account when considering a surgical management.
