Losartan Is Not Associated With Decreased Postoperative Arthrofibrosis After Hip Arthroscopy

PURPOSE: To determine whether losartan is associated with lower prevalence of hip adhesions, instability, and repeat procedures after hip arthroscopy and to assess this association among men, women, and patients under 30 years of age. METHODS: The TriNetX database was queried to identify all patients who underwent hip arthroscopy. Two cohorts were generated: one consisting of patients prescribed losartan after surgery and a comparison group excluding patients with any documented prescription for commonly used angiotensin-converting enzyme inhibitors or angiotensin-receptor II blockers. After 1:1 propensity score matching by age, sex, body mass index, essential hypertension, lower respiratory disease, nicotine dependence, diabetes mellitus, anemia, chronic kidney disease, heart failure, and metabolic disease, both cohorts included 947 patients. Rates of medical and orthopaedic complications were analyzed at 30, 90, and 180 days and at 2 years. A subanalysis was performed for patients under 30 years of age. RESULTS: Rates of lysis of adhesions were low at all time points in both cohorts of 947 patients. This held true for other orthopaedic complications. Rates of serious medical complications were low and had no significant differences between cohorts. Subanalysis of cohorts with women (627 patients), men (308 patients), and patients 29-years-old or less (388 patients) also showed similar rates of lysis of adhesions. CONCLUSIONS: Orthopaedic complications after hip arthroscopy are very rare regardless of whether or not postoperative losartan is prescribed. Serious medical complications were also rare in both cohorts. These results were consistent when stratified by sex and among patients 29-years-old or younger.
