SDB includes a spectrum of phenotypes defined by the type, nature and distribution of respiratory events across sleep stages, each presenting distinct clinical features and prognoses influenced by comorbidities. Proper phenotyping of SDB (through detailed event scoring, assessment of daytime ventilatory drive, and clinical profiling) enables tailored therapeutic strategies and improved outcomes. However, SDB is often oversimplified as sleep apnoea, overlooking its heterogeneity. This chapter explores methods for distinguishing central from obstructive hypopnoeas and underscores the importance of integrating physiological measurements such as the hypercapnic ventilatory response test, and clinical and patient-reported assessments in sleep laboratories, to personalise care and enhance prognosis for patients with SDB.
