Endovascular Management of a Giant Hepatic Artery Aneurysm

Hepatic artery aneurysms are the second most common visceral artery aneurysm, accounting for approximately 20% of cases. A 71-year-old female with hypertension and diabetes presented for back pain one month prior and was found to have an 8.0cm hepatic artery aneurysm. Endovascular repair was performed via common femoral artery access under local anesthesia. A 7 × 29 mm Gore VBX stent-graft, followed by a 7 × 39 mm VBX with 2 cm overlap. Completion angiography confirmed aneurysm exclusion with preserved flow to the liver. Despite the technical challenges of short landing zones, aneurysm exclusion was achieved with endovascular treatment.
