Last 5 Years’ Experience of Mini PCNL in Paediatric Population: Study from Kidney Dedicated Institute

Objective: To assess safety and efficacy of mini PCNL in pediatric population. Background: There has been increase in incidence and prevalence of nephrolithiasis globally with Mini PCNL emerging as a viable treatment option for pediatric patients. Standard PCNL is defined as using sheath size of 24 to 30 Fr. Whereas, Mini-PCNL encompasses a range of techniques using tract sizes from 4.8 to 22 F to treat renal stones. Mini-PCNL is only conducted at a few specialized centers in Pakistan. Due to specialized challenges and fear of results there is significant dread among pediatric populations. In this study, our aim is to evaluate the 5-year experience of mini-PCNL with the primary objective to assess stone free rate. Methods: A retrospective study was conducted at the kidney Centre hospital in Karachi, Pakistan, after approval for ethical review committee. It included the data of patients who underwent mini-PCNL for kidney stones between January 2017 and December 2021 at the Kidney Center, Karachi. Indications for mini-PCNL included lower pole stones >1 cm, kidney stones >2 cm, and stones that were unresponsive to shock wave lithotripsy (SWL). All the patients who were below 16 years of age and underwent mini-PCNL were included in it. Result: There were 210 participants in our study in which males were 142(67.6%) and females were 68(32.4%). The mean age was 6.67±3.85 years. The stone clearance rate was high in our patients, 155 (73.8%) Complications observed post-operative fever in 27(12.9%) and hematuria 9(4.3%) patients. Conclusion: Our study showed a lower than average clearance rate when compared with other countries due to a slightly stricter cut off criteria for the definition of stone free rate (2cm). The complication rates of performing mini PCNL in out setup were either in line with other complication rates of existing studies or lower.
