Integrative Ayurvedic Management of Amavata (Rheumatoid Arthritis): A Single Case Study

Background: Amavata, described as a Vata‑Kapha predominant disorder characterized by Ama and vitiated Vata lodging in Sandhi and other Kapha‑predominant sites, shares close clinical similarity with rheumatoid arthritis (RA). RA is a chronic, progressive, inflammatory polyarthritis affecting about 0.8% of the global population, with women being approximately three times more commonly affected than men, and leads to substantial impairment of quality of life. Objective: To evaluate the effect of a comprehensive Ayurvedic Shamana and local therapy protocol in a patient diagnosed with Amavata (RA). Methods: A single 25‑year‑old male patient with classical features of Amavata was treated with a combination of oral Ayurvedic formulations (Singhanad Guggulu, Rasnasaptakam Kwatha, Brihat Vata Chintamani Rasa) and local therapies (Dashang Lepa and Ruksha Swedana with Valuka Pottali). Clinical assessment was performed using grading scales for Sandhishoola (pain), Sandhishotha (swelling), Sparshasahatwa (tenderness), morning stiffness, and basic haematological parameters (haemoglobin, ESR, RA factor) at baseline and after 30 days. Results: Marked reduction in joint swelling and tenderness was observed within 10 days, with progressive improvement in pain and morning stiffness within the first week of treatment. By day 30, all major clinical symptoms had subsided, and ESR decreased from 40 mm to 5 mm at the end of the first hour, while RA factor remained reactive. Symptom‑free status was maintained during one month of follow‑up. Conclusion: The integrative Ayurvedic regimen used in this case effectively reduced the clinical manifestations of Amavata and improved functional status within 30 days, with sustained benefit over short‑term follow‑up. This case suggests that individualized Ayurvedic management may represent a promising complementary approach in RA, warranting further evaluation through well‑designed clinical studies.
