Objective. To characterize the pathogenesis of long bone defects resulting from modern combat trauma as a fundamental basis for developing treatment technologies and protocols for victims.Materials and Methods. The study is based on a retrospective analysis of long bone defects in 138 patients resulting from modern combat trauma. Parametric and nonparametric methods were used for data analysis.Results. Long bone defects most frequently resulted from mine-blast injuries—in 69.57% of the patients. In 20.65% of the victims, they were the result of shrapnel wounds, and in 9.78% of the victims—that is, the least frequently—they were the result of traffic accidents caused by combat operations. As a result of modern combat operations, long bone defects most often occur in segments of the lower extremity—in 86.96% of the victims. The incidence of long bone defects in the upper extremity segments is only 13.04%. In cases of mine-blast trauma, long bone defects occur in the lower leg in 75.00% of victims and in the thigh in 17.19%. Long bone defects resulting from mine-blast injuries occur in the upper limb in 7.81% of victims and in the shoulder in only 1.56% of victims. It has been demonstrated that damage to a particular limb segment is significantly influenced by the circumstances of the injury.Conclusions. The origin of long bone defects resulting from modern combat trauma is determined primarily by the circumstances of the injury and the anatomical characteristics of the damaged segment. Most often, long bone defects result from injuries caused by mine-blast trauma (69.57%), injuries to the distal segments of the limbs (81.25%), and injuries to the lower limbs (82.19%). There is a probable correlation between the circumstances of injury and the anatomical location of the injury in the genesis of long bone defects resulting from combat trauma.
