Efficacy of photodynamic therapy in the management of patients with VIN I–III

Photodynamic therapy (PDT) is an effective technique for treating precancerous conditions and cancers of various locations with a minimal risk of damaging normal tissues. The article presents results of using photodynamic therapy with chlorine-based photosensitizer Photoditazin in 50 patients with VIN I-III. The photosensitizer was administered intravenously at dose 1 mg/kg of body weight. PDT session was made in 2-2.5 hours after that. A diode laser with wavelength 662 nm was used as the radiation source. To deliver light to the pathological sites an end-light guide with macrolenses was used. Irradiation field dimensions ranged from 1 to 2 cm, and the number of fields from 1 to 4, light energy from 100 to 150 J/cm², power density from 0.23 to 0.31 W/cm². PDT session lasted for 8 - 32 minutes depending on the number of irradiation sites. Response of the pathological focus to the treatment was assessed immediately after PDT session, and then in 1, 7, 14, and 30 days; dynamic follow-up of patients was for one year. After treatment, complete regression was achieved in all patients with dysplasia grade I–II, in 96% of patients with dysplasia grade III, and in 60% of patients with carcinoma in situ (CIS). During 12-month follow-up , no recurrences were registered in any of the patients. Photodynamic therapy is an alternative method for treating VIN I-III. Moreover, after PDT normal anatomy of the vulva is preserved, functions of the affected organ remain intact, and no cosmetic defects develop.
