Cough frequency monitors: validated in research but absent from the market?

Background: Objective cough assessment is recommended for evaluating antitussive therapies, yet no objective method of cough monitoring is widely adopted. Aims: To evaluate cough frequency monitors used in research studies and identify commerciall availability and associated factors such as funding. Methods: A systematic review was conducted in accordance with PRISMA-DTA guidelines and registered in PROSPERO (CRD42020187919). Analysis was based on cough monitor and performance. Studies were classified by funding source, and subgroup analyses were performed. Funding source, and commercial availability was explored through a descriptive, ad hoc analysis of the literature, via targeted web searches and manufacturer contact. Results: Eleven cough frequency monitoring devices and four mobile applications were included. None was found to be commercially available. Lifeshirt was the only system originally developed for commercial use, but has been unavailable since company liquidation. VitaloJak and the Leicester Cough Monitor are available exclusively for research purposes, while no response was obtained regarding Pulmotrack-CC. Most validation studies included small samples, and methods used to calculate diagnostic accuracy were often insufficiently described. Meta-analysis demonstrated high sensitivity and specificity in both privately funded (93% and 99%, respectively) and publicly funded (88% and 99%, respectively) studies. Conclusions: Despite technological advances and high diagnostic performance, cough frequency monitors have not translated into routine clinical practice, underscoring barriers related to implementation and commercialization.
