A case of giant peripheral odontogenic fibroma arising in the maxillary gingiva

Odontogenic fibroma is a rare odontogenic tumor of mature fibrous or fibromyxoid connective tissue and occurs in the central (intraosseous) or peripheral (extraosseous) region.We report a case of giant peripheral odontogenic fibroma in the maxillary gingiva.A 66-year-old female was referred to our department with a chief complaint of a maxillary gingival swelling with bleeding and difficulty in eating.At the initial examination, a 75 × 60 × 37 mm sessile mass with redness and induration was observed on the right maxillary gingiva.CT and MRI showed no apparent bone destruction or lymph node swelling.Histopathology of the biopsy showed no evidence of malignancy.Based on a clinical diagnosis of maxillary tumor, tumor resection, including the surrounding alveolar bone, was performed under general anesthesia.The excised specimen was a multilobular mass with a solid, uniformly yellowish-white cut surface.Histopathological findings revealed the proliferation of spindle-shaped cells, resembling fibroblasts, within a collagenous connective tissue, along with cords of epithelial masses, leading to the diagnosis of peripheral odontogenic fibroma.Approximately six years have passed since the surgery, and no recurrence has been observed..
