Effect of Ultrasound-Guided Erector Spinae Plane Block on Intraoperative Blood Loss in Lumbar Fusion Surgery: Randomized Controlled Trial

Objective: This study aimed to evaluate the effect of the erector spinae plane block (ESPB) on intraoperative blood loss in patients undergoing lumbar fusion surgery. The effects of ESPB on surgical field visualization, remifentanil consumption, and early postoperative pain control were also examined. Material and Methods: Patients aged >18 years with American Society of Anesthesiologists physical status I-III scheduled for elective posterior lumbar spinal fusion involving up to 2 levels for degenerative spinal disease were included. Patients were randomized to receive either ultrasound-guided bilateral ESPB or general anesthesia alone (control group). Intraoperative blood loss, transfusion requirements, hidden blood loss, hemodynamic parameters, surgical field visualization, surgeon satisfaction, remifentanil consumption, operative time, and length of hospital stay were recorded. Results: A total of 120 patients were enrolled, with 60 assigned to the ESPB group and 60 to the control group. Intraoperative blood loss was lower in the ESPB group (300 [50-600] mL vs. 400 [50-800] mL, p=0.009). Surgical field visibility scores were also lower in the ESPB group at all intraoperative time points (p<0.05). Remifentanil consumption was lower in the ESPB group (1195.8±256.6 µg vs. 1321.9±272.4 µg, p=0.01). The duration of surgery was shorter in the ESPB group (135.5 [120-157] min vs. 140 [125-165] min, p=0.03). Early postoperative NRS scores were lower in the ESPB group (p<0.001). Conclusion: ESPB reduced intraoperative blood loss, improved surgical field visualization, and enhanced early postoperative pain control in lumbar fusion surgery. It may be a useful adjunct in perioperative anesthetic management.
