PREVENTABLE ADVERSE EVENTS IN HEALTHCARE SERVICES: SYSTEMIC WEAKNESSES, SAFETY CULTURE AND MULTIPROFESSIONAL STRATEGIES FOR REDUCING HEALTHCARE-RELATED HARM

Preventable adverse events remain one of the greatest challenges to healthcare quality and patient safety worldwide. Despite important advances in clinical protocols, public policies, and risk management strategies, patient harm associated with healthcare delivery continues to represent a significant concern, particularly in high-complexity care settings. This study aimed to analyze the systemic weaknesses associated with preventable adverse events, discussing the influence of patient safety culture and multiprofessional strategies in reducing healthcare-related harm. A qualitative, descriptive, and analytical integrative literature review was conducted using studies retrieved from PubMed/MEDLINE, Scopus, Web of Science, ScienceDirect, the Virtual Health Library, and Google Scholar, in addition to technical documents from national and international health organizations. The evidence showed that excessive workload, communication failures, fragmented care, insufficient human resources, and weak organizational safety cultures are among the main factors contributing to preventable adverse events. Conversely, healthcare organizations that promote a strong patient safety culture, effective communication, organizational learning, and collaborative multiprofessional practice demonstrate better quality indicators and lower rates of preventable harm. Clinical protocols, surgical safety checklists, incident reporting systems, and clinical decision-support tools were also identified as effective strategies for strengthening risk management and improving healthcare quality. It is concluded that reducing preventable adverse events requires the integration of institutional policies, organizational processes, and collaborative healthcare practice, recognizing patient safety as a continuous and shared responsibility across all levels of healthcare delivery.
