Clinical reasoning pathways of nurses during Nursing Process simulation: de-identified analytical data and methodological documentation

This dataset contains the de-identified analytical data and methodological documentation supporting the study Clinical reasoning pathways of nurses during Nursing Process simulation. The study examined how nurses maintained or lost coherence across successive stages of the Nursing Process during an individual high-fidelity simulation scenario. Twenty-six nurses completed the simulated activity. Two complementary sources of evidence were used: Nursing Process documentation recorded in a simulated electronic health record (sEHR) and audio-video recordings of participants’ clinical performance. The validated clinical case and simulation scenario were operationalized through a 58-item answer key covering assessment, diagnosis, planning, implementation, and evaluation. Participant-level evidence from the sEHR and video recordings was reviewed and integrated at the item level, generating a long-format analytical matrix with 1,508 participant-item combinations, of which 1,492 received a binary final classification and 16 diagnosis-related combinations were classified as not applicable because two participants did not record a nursing diagnosis. The repository includes: the 58-item bilingual answer key; the de-identified participant-by-item analytical matrix; participant characteristics; multiple-response specialty data; decision-tree node and complete pathway frequencies; participant-level inputs and exact results for the McNemar paired comparisons; participant-level stage summaries and pathway classifications; methodological documentation describing the clinical situation, simulation scenario, evidence sources, triangulation procedures, coding rules, and pathway reconstruction; a data dictionary; an item-level provenance map; and a verification report. Clinical reasoning pathways were reconstructed from observable clinical actions and documented decisions rather than treated as direct measurements of an underlying psychometric construct. The decision-tree framework was used to examine coherence between assessment data, nursing diagnosis, prescription/planning, implementation, and final pathway closure. The package is intended to support transparency, methodological appraisal, secondary examination of the analytical structure, and reproducibility of the descriptive and paired analyses reported in the associated manuscript. Audio-video recordings and raw sEHR exports are not included because they contain identifiable images, voices, clinical documentation, and professional-performance information.
