Introduction: There has been no significant change in the treatment of thromboangiitis obliterans (TAO) for the past 20 years. Low-intensity pulsed ultrasound (LIPUS) has the potential for promoting angiogenesis and can be used as a treatment option for TAO.Methods: The purpose of this study is to evaluate the effects of LIPUS on chronic limb-threatening ischemia (CLTI) and clinical parameters in patients with TAO. A total of 23 “no-option” TAO patients with persistent CLTI for more than 3 months were enrolled. Patients with CLTI were defined as patients having severe rest pain without ulcer/gangrene, with localized ulcer/gangrene, and advanced ulcer/gangrene. Clinical parameters were evaluated at baseline and after 12 weeks of treatment with LIPUS.Results: Rest pain intensity on a visual analog scale, walking impairment, skin perfusion pressure, flow-mediated vasodilation, nitroglycerine-induced vasodilation, Rho-associated kinase activity, and high-sensitivity C-reactive protein were significantly improved after 12 weeks of LIPUS treatment. During a median follow-up period of 94.8 months, there were 3 major amputations and 7 minor amputations. The rates of major amputation free survival were 91% at 1 year and 85% at 5 years, and the rates of minor amputation free survival were 78% at 1 year and 73% at 5 years. There was no report of adverse effects associated with LIPUS.Conclusion: LIPUS may be a promising adjunctive therapeutic option for clinical symptoms, ameliorating inflammation, and improving vascular function in TAO patients with CLTI.
