Duodenal Injury in Resource-Scarce War Setting: A Case Report from Gaza

Duodenal injuries are rare, but they are severe traumatic injuries that carry high morbidity and mortality. Management of such traumatic injuries poses a challenge to surgeons, more profoundly in resource-constrained settings. This case aims to highlight the importance of critical surgical decision-making and the application of damage control surgery for life-threatening injuries in vulnerable populations with limited access to labs, imaging, basic medications, and intensive care services, when healthcare systems have collapsed. This report describes the presentation, management, and successful outcome of a grade 3 duodenal injury in a 5-year-old boy who suffered multiorgan damage due to the blast impact of a tank missile hitting his home in Mawasi Khan Younis, Gaza in February of 2024. There is a dire need to have special surgical protocols tailored to conflict zones. This is to ensure appropriate application of damage control surgery can provide the best outcome, particularly in settings of healthcare collapse and resource deficiency. This report also highlights the need for greater international aid and global support to end wars and support the vast casualties of civilian populations caught in the tragic crossroads of war and devastation.
