Introduction: The GRADE (Grading of Recommendations Assessment Development and Evaluation) working group brings together engaged professionals to develop an optimal system for assessing the certainty of the evidence derived from systematic reviews, as well as for health care technologies, and to determine the strength of clinical practice guideline recommendations. Objective: This article provides an introduction to the GRADE approach and to the evidence profile and summary of findings tables for presenting evidence certainty (quality or confidence) ratings. These tables can be used in the context of systematic reviews as well as in health care technology assessments and clinical practice guidelines covering different options for treatment, diagnosis, values and preferences, among others. Methodology: The GRADE process begins with a question which specifies all the outcomes that are important to the patient. After gathering and summarizing the evidence for each outcome, GRADE offers explicit domains to rate the certainty of the evidence, including study limitations, imprecision, inconsistency, indirectness and publication bias. Results: The certainty of the evidence for each outcome is rated as high, moderate, low or very low. In the case of recommendations, these are classified as strong or conditional (formerly weak), and in favor or against. Evidence profile tables are used to report the assessment of each of the GRADE domains as well the certainty ratings for every outcome. Summary of findings tables report the final result of the certainty evaluation process for each outcome. Conclusions: GRADE recommends assessing the certainty of the evidence for each outcome that is important to the patient and presenting this assessment succinctly in transparent and informative evidence tables.
