Open Science Self-Evaluation Form

This self-evaluation form is based on the Open Science Career Assessment Matrix (OS-CAM) and is designed to help researchers reflect on their engagement with and contributions to Open Science practices. For each activity area—covering Research Output, Research Process, Service and Leadership, Research Impact, Teaching and Supervision, and Professional Experience—users assess their involvement using a five-point Likert scale (“Strongly disagree” to “Strongly agree”) and can add comments noting achievements, challenges, or plans for growth. The form encourages honest self-assessment to identify strengths to build on and areas needing development. At the end of the form, users will find curated resources tailored to different experience levels to support further growth in Open Science.
