Semaglutide-induced nonarteritic anterior ischemic optic neuropathy

Semaglutide, a glucagon-like peptide-1 (GLP-1) receptor agonist widely used for type 2 diabetes and obesity management, has recently been associated with vision impairment, particularly nonarteritic anterior ischemic optic neuropathy (NAION). This case series describes the clinical course of three patients who developed acute, painless vision loss shortly after initiating semaglutide therapy. Clinical findings included optic disc edema, peripapillary hemorrhages, and visual field defects, confirming the diagnosis of NAION. Management involved drug discontinuation and supportive therapy. A review of existing literature and pharmacovigilance data suggests semaglutide may carry a higher risk of vision impairment than other GLP-1 agents, especially in patients with vascular risk factors such as diabetes, hypertension, obesity, and hyperlipidemia. While causality is unproven, the temporal relationship warrants attention. Ophthalmologists and physicians prescribing semglutide should maintain a high index of suspicion and consider ophthalmic monitoring and follow-ups in patients using semaglutide, pending further research on this potential adverse effect.
