Fever of Unknown Origin Solved With Novel Diagnostic Tool

A 51-year-old White man with a 2-year history of fever of unknown origin (FUO) presented for further evaluation after extensive, unrevealing work-up both in inpatient and outpatient settings at multiple centers, with ongoing weight loss and malnutrition. Despite negative gastrointestinal biopsies, cell-free DNA next-generation sequencing testing identified Tropheryma whipplei, leading to the diagnosis of Whipple disease confirmed by lymph node biopsy. Symptoms resolved with antibiotic treatment. This case highlights the importance of continued investigation with novel diagnostic tools, such as cell-free DNA next-generation sequencing testing, in patients with FUO who continue to clinically deteriorate.
