Adhesion-reducing effects of meloxicam and hyaluronic acid/carboxymethyl cellulose barrier in rat hepatectomy model

Objective : Postoperative peritoneal adhesions substantially increase the complexity and risk of repeat liver resections. This study evaluated the anti-adhesive efficacy of a hyaluronic acid/carboxymethyl cellulose barrier (Seprafilm®), Meloxicam (10 mg/kg), and their combination in a rat hepatectomy model. Methods : 28 male Wistar rats underwent a 40% partial hepatectomy and were randomized into four groups (n=7): Control, Meloxicam, Seprafilm®, and Combined (Meloxicam + Seprafilm®). On postoperative day 7, adhesions were evaluated using the Modified Linsky macroscopic scale and semi-quantitative histopathological grading. Results : Macroscopic adhesion scores were significantly lower across all treatment groups compared with the control group ( p <0.001). The combined treatment group had a significantly lower extent of adhesions than the Seprafilm® group alone ( p =0.027). Histopathological analysis showed no significant intergroup differences. Notably, clinical complications, including wound dehiscence and early mortality, were observed in the high-dose treatment groups, suggesting potential interference with early tissue repair. Conclusion : The Meloxicam group and the Meloxicam and Seprafilm® combination effectively prevent macroscopic adhesions after hepatectomy, whereas Seprafilm® alone shows a favorable numerical trend toward reduction (p=0.064). However, the 10 mg/kg meloxicam dose is associated with a narrow safety margin for wound healing.
