‘Community perspectives on long-acting injectable PrEP among Haitians and Haitian Americans living in Florida: a qualitative study’

Introduction: Long-acting injectable (LAI) pre-exposure prophylaxis (PrEP) options for HIV prevention are becoming increasingly available. Haitians and Haitian Americans in South Florida (FL) face a disproportionate HIV burden but have low adoption of prevention tools. Theory: This study provides insight into PrEP preferences and explores attitudes towards LAIs (specifically cabotegravir) from the perspectives of Haitian and Haitian American community members and key stakeholders (community members with leadership roles that can impact systems) in FL, using a community-engaged approach. Methods: From August to November 2024, semi-structured qualitative interviews were conducted with 15 Haitian community members and 15 stakeholders. Data were analyzed using an inductive thematic analysis approach, with a codebook to guide coding and theme identification. Results: Three primary themes emerged (1) Perspectives diverged, with community members emphasizing health protection, while stakeholders highlighted stigma as a major barrier to LAI-PrEP uptake, (2) community members preferred LAI-PrEP for its convenience and reduced adherence burden, and (3) interviewees identified access and education as greater barriers than stigma or mistrust. Discussion: Expanding LAI-PrEP uptake requires delivery and linkage strategies that reduce access barriers and strengthen targeted education efforts.
