Abstract Magnetic resonance–guided radiotherapy (MRgRT) provides high-contrast soft-tissue imaging, continuous or near-real-time visualization of anatomical structures, and online adaptive treatment planning, demonstrating significant advantages over conventional computed tomography-guided radiotherapy in target localization accuracy, organ motion management, and dosimetric controllability. Meanwhile, the strong static magnetic field leads to deep coupling among the magnet, accelerator, and radiation transport, posing a series of unique challenges in system engineering and medical physics. Focusing on magnetic resonance linear accelerator (MRI-LINAC) systems, this paper reviews the current status and research progress of key MRgRT technologies from the perspectives of superconducting magnet technology, linear accelerators, treatment planning systems, and medical applications. Finally, future development trends in magnet engineering, dose transport modeling, and clinical application expansion are discussed.
