Genital Psoriasis: An Updated Review of Diagnosis and Management for Healthcare Providers

Psoriasis is a chronic inflammatory skin condition that may affect all body areas, including the genital area. Genital psoriasis occurs in up to two‐thirds of patients with psoriasis at some point during their lifetime but is often overlooked. Genital psoriasis is associated with severe impairment in sexual health and quality of life. Despite the prevalence of genital psoriasis, there is often a lack of communication between patients and healthcare providers, resulting in underdiagnosis and under‐reporting of genital psoriasis along with lack of treatment or inappropriate self‐treatment. There are only a limited number of interventional clinical trials evaluating the efficacy and safety of treatments. The current initial treatment recommendations include topical corticosteroids, vitamin D analogues, phosphodiesterase‐4 (PDE4) inhibitors, and topical calcineurin inhibitors. Systemic and biologic therapies are reserved for unresponsive and/or widespread disease. Moreover, the genital skin is susceptible to increased absorption and adverse effects from current topical therapies, making treatment of genital psoriasis challenging. Patients with genital psoriasis may present to clinicians with varying expertise in the diagnosis and management of genital psoriasis. To improve management of patients with genital psoriasis, there is an unmet need for sharing of expertise and partnership between nondermatology healthcare providers and dermatologists.
